Pour coffee into mug and stir in peppermint extract if using unflavored. Add sweetener if desired (natural options like maple syrup, honey, agave, or stevia for low-sugar recommended). Place milk in large microwavable jar and heat for 30-45 seconds. Stir in cocoa powder, then use a small hand blender to froth.
